Blameless Postmortem Framework
Blame kills learning. Our postmortem template focuses on systems and processes, not individuals. Sections cover: incident summary, user impact quantification, timeline of events, contributing factors (not "root cause" singular — incidents are always multi-causal), what went well, what could be improved, and concrete action items with owners and deadlines.
Timeline Reconstruction
We set up tooling to automatically generate incident timelines from your alert history, deployment logs, and communication channels. Instead of relying on memory during the review meeting, participants see a chronological view of every alert, deploy, rollback, and Slack message. This catches details that would otherwise be forgotten or misremembered.
Action Item Tracking
The postmortem is worthless if action items rot in a document. We integrate postmortem outputs into your issue tracker — Jira, Linear, GitHub Issues — with severity-based SLAs. Critical action items get a 7-day deadline. Improvement items enter the next sprint planning. A monthly review flags overdue items and escalates to engineering leadership.

Postmortems are indexed and searchable. When a new incident occurs, responders can search past postmortems for similar symptoms. Quarterly reliability reviews aggregate trends across incidents: most common contributing factors, services with repeated failures, and the ratio of completed to outstanding action items.
